Academic Forms

 

Below are academic forms according to a suggested timeline for each degree. The following steps for each are Graduate School requirements for completing your degree. Your program may have additional requirements.

Complete and submit all forms and requests to your program coordinator for review. The program coordinator will get the signature of the Director of Graduate Studies (DGS) and submit the forms to the Graduate School.

Complete Coursework. Check with your program regarding transfer credit and independent study policies.

Submit intent to graduate.

Spring graduate: February 28, 2019

3.

Prepare thesis according to the Thesis and Dissertation Guidelines.

   

4.

Schedule an appointment to review thesis format.

5.

Submit signed title page and abstract to the Graduate School, along with Completion of Master's Thesis form.

Upload final thesis to ETD site (Electronic Theses & Dissertations).  

SPECIAL NOTE RE: FINAL COPY OF THESIS AND DISSERTATION:  When a thesis or dissertation is approved by committee members, and the format has been reviewed by the Graduate School, this becomes the “final copy.” After it has been uploaded to the ETD website and approved, changes cannot be made to this document.

11. 

Submit dissertation defense results to the Graduate School.

Make final content revisions to dissertation, as requested by the committee.
