With the exception of individual study requests, please submit all academic forms to the Graduate School Administration Submissions Portal. Below are academic forms according to a suggested timeline for each degree:
Master’s Degree
- Complete Coursework. Check with your program regarding transfer credit and individual study policies.
- Transfer credit worksheet
- Request to receive graduate credit for an undergraduate course
- Request for Individual Study – students initiate this process on their student landing page in YES
- Permission to audit
- Request to appoint or change adviser
- Missing grade form for administrators
- Submit Intent to Graduate
- Students planning to graduate at the end of the fall, spring, or summer term should submit the Graduation Intent form to the Registrar’s Office through the YES student portal.

- The Graduation Confirmation User Guide provides additional instructions required for graduation.
- Students planning to graduate at the end of the fall, spring, or summer term should submit the Graduation Intent form to the Registrar’s Office through the YES student portal.
- Prepare Thesis according to the Thesis and Dissertation Guidelines
- Submit your thesis as a primary document and your signed title page and abstract as administrative files in VIREO.
- Complete Coursework. Check with your program regarding transfer credit and individual study policies.
Ph.D. Program
- Alternative to pdf forms, graduate students can utilize the Doctoral Workflow form to request to:
- Appoint/modify Ph.D. committee
- Schedule/modify qualifying exam
- Schedule/modify dissertation defense
- Extend deadline to Enter Candidacy
- Extend Candidacy

- Dissertation Defense and Submission
- Request to schedule dissertation defense
- Take copy of the title page and abstract to defense to get signatures
- Request to cancel or reschedule dissertation defense
- Your adviser or department administrator will submit dissertation defense results to the graduate school administration submissions portal. Submit your dissertation, signed title page and abstract, your CV and your SED completion certificate to your VIREO profile. More information can be found here.
